Registration



Registration for Spring Season
Location: Elizabeth Forward Middle School Cafeteria
Dates: January 14th, 19th, 21st and 27th
Time: 6:30pm to 8:00pm

ALL registrations after January 27th will be considered late registrations being subject to late fees and will only be accepted by mailing the completed registration form and payment in full to the EFSA P.O. box.

Please remember to bring any outgrown and gently worn uniforms and equipment with you to donate and help defer costs for those families that need some help. This program has had a great start and only works if you donate. For those looking for donated uniforms and equipment please register early as this is a first come first serve program.


1st Time Players!!!!

Players registering for the first time are required to show proof of age
(Birth Certificate).


Traveling Players
MUST provide the following at time of registration or you will NOT be permitted to register!!   NO Exceptions!!!
  Copy of Birth Certificate
  Last 6 Digits of Social Security Number
  Small 1x1 school photograph with players name written on the back

  Family: $35.00 Buy-Out   OR  FUNDRAISER !!
   Each family must purchase 8 raffle tickets @ $5.00 each ($40) and then sell them outright. Ticket stubs to be returned at a later date.
  All players MUST participate in a fundraiser or buy-out.

In House Reversible Jersey $20.00
Traveling Registration $50.00
Traveling Uniform $20.00 for 2 T-shirts


A $20.00 Late Fee will be imposed on all registrations received after the last registration sign-up. No Exceptions! Acceptance of Late Registrations are subject to team availability and board approval! Signed registration form and all fees must be submitted to be eligible.
